LifeSpan RE Permissions, Rules, & Waiver at First Parish Milton

Participation Expectations & Rules 

To help ensure a safe and positive experience for everyone, participants are expected to follow these rules:

1. Check In & Check Out
   • When not actively participating in a RE program, guardians are responsible for supervising all minors in their care.
   • Children and youth must check in at the beginning of each event and check out before leaving.
2. Behavior Expectations
   • Children and youth must follow the directions and rules of staff, chaperones, and event venues.
   • Participants who violate directions, rules, or laws may be sent home at the discretion of staff and at the expense of their parents or guardian.
3. Transportation Changes
   • If pickup or transportation plans change during an event, a parent or guardian must notify staff by text, phone call, or email approving the change.
4. Safe Environment Policy
   • All youth and adult volunteers agree to follow the church’s Safe Environment Policy.
   • A copy of this policy is available upon request from church staff.

Photo, Audio Video Permissions

I understand that First Parish Milton may take photographs, audio recordings, and videos of participants for use on the church’s website, social media, printed materials, and other communications. Unless I notify the Director in writing before the event, I consent to this use.

Transportation Permissions

Some activities involve transportation and participants may ride in a personal vehicle of another participant, staff member, or volunteer. Unless I notify the Director in writing before an event, I consent to this arrangement.

Medical Policy Permissions

I understand that I am financially responsible for any medical expenses incurred as a result of participation.

Unless I notify the Director in writing before the event, I authorize designated staff and volunteers to:

1. Administer basic first aid for minor injuries or illnesses.
2. In the event of an accident or illness, act on my behalf to consent to medical treatment deemed advisable by licensed medical professionals.
3. Provide medications or medical assistance as specifically authorized in writing by a guardian.

Permission, Waiver, & Release of Liability

By indicating consent (such as signing or completing an online form that refers to this document), I acknowledge that:

• I am either the adult participant, or the legal guardian of the participant(s), and I voluntarily choose to participate, or permit the participant(s) to participate, in First Parish Milton Religious Exploration activities, including transportation related to these activities when applicable.
• I waive the right to bring legal action against the church, its employees, officers, volunteers, and agents for claims arising from participation in these activities. I release the church and its representatives from liability for injuries, losses, or damages resulting from participation, except where prohibited by law. If I am the legal guardian of a minor participant, I accept responsibility for that participant’s actions and obligations. I acknowledge that participation involves both known and unknown risks, and I voluntarily assume those risks. I agree to hold harmless and indemnify the church against any claims, losses, damage to personal property, liabilities, costs, and reasonable attorney’s fees arising from participation in these activities.
